Citizen Comment Forms for Addressing Advisory Boards & Commissions

To address an Advisory Board on a posted item, complete the Request to Address Bd or Comm for a posted item half sheet and return it to the Board Liaison prior to the discussion of the item at the Advisory Board meeting. These forms are also provided to you at the Advisory Board meeting.

If you would like to address the Advisory Board regarding an issue that is not posted on the agenda, please fill out the Request to Address the Bd or Comm for a non-posted item and submit it to the Board Liaison no later than one week prior to the Advisory Board meeting.

Conflict of Interest Forms for Advisory Board & Commission Members

According to the City’s Ethics Ordinance, an Advisory Board Member must file a Ch. 171_Conflict of Interest Affidavit or Ch. 2.20_Conflict of Interest Affidavit with the Board Liaison’s Office prior to recusing themselves from action at a board meeting.

Section 2.20.030 in the Code of Ordinances relates to the Disclosure of Interest and Recusal —

A City official who has a substantial economic interest in the outcome of the matter under consideration shall disclose that the official has a substantial economic interest and shall recuse himself/herself from discussion of and voting on the matter. The City official shall also promptly file an affidavit with the City Secretary (or Board Liaison) disclosing the nature and extent of the conflict, and the affidavit shall be included in the official minutes of the meeting body.

A City official having a substantial financial interest in the outcome of a matter under consideration shall disclose that the official has a substantial financial interest and recuse himself/herself immediately from voting and from the discussion of the matter. The City official shall also promptly file an affidavit with the City Secretary (or Board Liaison) disclosing the nature and extent of the conflict, and the affidavit shall be included in the official minutes of the body.
